Getting Your Results

You can now get your screening test results directly from Eurofins NTD, LLC.

* To have your test results sent to you, please download the Request for Healthcare Records and Authorization to Release Healthcare Information form or call us at 1-888-NTD-LABS (683-5227) to request the form. Please complete this form and return it to us, along with a valid photo ID and other supporting documentation (listed below), in one of the following ways:

After receiving your completed form and supporting documentation, we will send you your test results within 30 days. We will charge 75 cents per page in response to a records request.

Or, if you prefer, you can request your test results by contacting your physician.

Important Instructions:

  1. Please complete all fields of the request form. Any missing information may result in an invalid form or a delayed response. If you have questions regarding the form, please call us at 1-888-NTD-LABS (683-5227)
  2. Each requestor will need to provide a photocopy (front and back) of a valid photo ID (see instructions below).
  3. If the person requesting the results is not the patient, then the requestor will need to also provide proof they are an authorized representative or designated party of the patient, e.g., parent, power of attorney, etc. (see below).

Acceptable Forms of Photo ID

To better protect your privacy, we ask that you submit a photocopy of a valid photo ID (front and back), along with your request form. If another person is requesting the information on behalf of the patient, the requestor should also submit a copy of his or her photo ID. If you are an attorney requesting results on behalf of your client, please submit a copy of your valid Bar License in lieu of one of the IDs listed below. Below is a list of acceptable forms of ID:

  • U.S. passport
  • U.S. passport card
  • DHS "Trusted Traveler" cards (Global Entry, NEXUS, SENTRI, FAST)
  • U.S. Military ID (active duty or retired military and their dependents, and DOD civilians)
  • Permanent Resident Card
  • Border Crossing Card
  • DHS-designated enhanced driver's license
  • U.S. driver's licenses or other state photo identity cards issued by Department of Motor Vehicles (or equivalent) for the sole purpose of identification.
  • Native American Tribal Photo ID
  • HSPD-12 PIV Card
  • An airline or airport-issued ID (if issued under a TSA-approved security plan)
  • Foreign government-issued passport
  • Canadian provincial driver's license or Indian and Northern Affairs Canada (INAC) card
  • Transportation Worker Identification Credential (TWIC)

Note: A weapon permit is not an acceptable form of identification.

Information for Authorized Representatives and Designated Parties

Please indicate on page 2 of the Request for Healthcare Records form if you are an Authorized Representative of the patient and are requesting the records on behalf of the patient.

1. Authorized Representatives. An Authorized Representative is a person who has the legal authority to act on behalf of another person by designation of the patient or a judge. An example would be a Power of Attorney.

If you are an Authorized Representative you will need to provide copies of photo identification for both the patient and yourself, except as noted below. You will also need to provide documentation demonstrating your legal authority to represent the patient in making a request for records on the patient’s behalf. Such documentation may include an executed Power of Attorney document.

2. Parents or Legal Guardians. If you are the parent of an un-emancipated minor or the Legal Guardian, you will need to provide copies of photo identification for you and the patient. You will also need to provide documentation demonstrating your legal authority to represent the patient in making a request for records on the patient’s behalf. Such documentation may include the patient’s birth certificate, where you are listed as the parent or documentation showing your Legal Guardianship.

Please note - photo identification will typically not be required for un-emancipated minors whose records are requested by a parent and/or legal guardian; however, Eurofins NTD, LLC. reserves the right to request photo identification of the minor patient based on the circumstances surrounding the request.

3. Attorneys. An Attorney who is submitting a request on behalf of a client should submit a copy of his or her active bar status, such as a Bar Admittance Card, in addition to paperwork signed by the client authorizing the attorney to represent the client in legal matters and/or to request the patient’s tests results and billing information. Attorneys should also submit a copy of the patient’s photo identification with the request.

4. Designated Parties. A Designated Party is someone who has been identified by the patient as someone who is able to receive access to the patient’s test results and billing information. Please note that the Designated Party will need to provide copies of acceptable forms of photo identification for both the patient and the Designated Party. The Designated Party to whom the records will be released should indicate what his or her relationship to the patient is, in case further follow up is required. The Designated Party will also need to provide notarized documentation indicating that the patient has authorized the release of his or her records to the Designated Party.
